Regardless of the financial status of the family, any child with special needs is entitled to receive services from the case management unit.
The Case Management Unit serves as the single point of entry into the health care system for Warren County families of children with special needs and for infants and toddlers receiving early intervention services
A Case Manager will:
- Assess each child, (birth to age 21), to determine how needs are being met and discuss ways to fulfill unmet needs.
- Provide information about NJ Family Care Program.
- Provide information about the Catastrophic Illness in Children Relief Fund Program.
- Develop and review an Individual Service Plan.
- Refer to appropriate federal, state, and local resources.
- Coordinate and monitor the delivery of services.
- Serve as an advocate for the child and family.
