Special Child Health Services Case Management Unit works with families to help meet the needs of children, from birth through 21 years of age, who are medically involved or developmentally disabled regardless of financial status.
The Case Manager is either a Registered Nurse or licensed social worker who works directly with the family to identify the child’s needs and to develop an Individual Service Plan based on those needs. All plans and goals are updated regularly, as the child’s progress is monitored. The Case Manager also helps to identify sources of assistance, financial and other, and helps the family to obtain services needed. There is no cost to families for Case Management services.
The “Advocating for Your Child with Special Needs” Series of workshops at the Bridgewater Library is a joint venture between the Somerset County Library and Somerset County Special Child Health Services Case Management Unit. Programs are designed for parents and other adults interested in children with special needs.
These workshops are offered on a complimentary basis and are open to the public. 1.5 professional development hours are awarded to teachers who attend.

All therapists at the Somerset Children’s Center are NJ licensed and highly trained in their specialties. Children are seen individually by a therapist who is uniquely qualified to help them reach their maximum potential. The treatment plan is comprehensive and personalized to meet the needs of each individual child. We strongly believe in a partnership between the families and our therapists to ensure success and facilitate the child’s progress.
