Our board-certified family physicians are specially trained to care for every member of your family, from newborns to adults. We work closely with you to develop an individualized care plan that may focus on disease prevention, treating illness, collaborating with specialists when needed, and coordinating care across the healthcare system.
Our primary care physicians are specially trained to be your first point of entry into the healthcare system and will be your advocate in coordinating all healthcare needs. You and your primary care physician will work — as a team — to meet your medical, physical, mental, and emotional healthcare needs.
We provide preventive care and comprehensive services for infants, children and adults :
Routine wellness checks (physical examinations, lab tests, vaccinations, gynecological care, pulmonary and cardiac testing, vision/hearing screenings, mental health assessment, nutritional counseling, and health education)
Chronic disease management (ie: diabetes, arthritis, obesity, osteoporosis, high blood pressure, Lyme disease, etc.)
Sick visits (colds, flu, infections, rashes, earaches, abdominal issues, etc.)
Minor injuries (including aches and pains, muscle pulls, sprains, sunburn, lacerations, etc.)
Minor surgical procedures (suturing, biopsies, wart removal, etc.)
Sports medicine injury care and joint injections. (Dr. Chen)
Integrated Behavioral Health Services
People with chronic health conditions like diabetes or high blood pressure may struggle to manage their own health at home, especially when they also suffer from anxiety or depression. Hunterdon Health offers patients an Integrated Behavioral Health model of care delivery in their primary care practices.
Integrated Behavioral Health is a very focused short-term treatment in the primary care practice, and connects patients to other resources throughout Hunterdon Health and in the community when necessary. The primary care doctor also has direct access to psychiatrists for assistance when needed.
