The mission of CCCC is to empower children and families through the provision of individualized care that promotes the maintenance of healthy and independent family functioning. We believe that children and their families are remarkably resilient and are more than capable of positive growth and development when provided with effective community-centered service and support.

Capitol County Children’s Collaborative (CCCC), an independent, non-profit care management provider is located in Mercer County, New Jersey. We are part of a statewide effort to ensure that all children have access to the full array of resources available. Each county in New Jersey is currently served by their own Care Management Organization (CMO).

Our Services

• Serve youth/families that are experiencing emotional, behavioral, substance use disorders, intellectual and/or developmental disabilities and chronic medical health challenges.
• Provide care coordination and wraparound care planning for children and their families with the most complex needs and are responsible for facilitating access to a full range of treatment and support services
• Facilitate, coordinate and work within child-family teams to develop individualized service plans (ISP) and connect to community sustainable resources.
• Keep children in their own homes, schools and in their community. We provide direct, face-to-face care coordination and wraparound care planning for children and their families with the most complex needs (mental health, behavioral, developmental delays, substance use disorder, and chronic medical challenges.
• Responsible for facilitating access to a full range of treatment and support services. Linkage to the community.
