Atlantic Visiting Nurse supports patients recovering from illness or surgery, those coping with chronic conditions, families facing end-of-life care, and older adults needing help to remain living independently.

Home Health
We help patients regain health, stay out of the hospital, and remain living independently at home. Our personalized care supports patient’s recovery from surgery or illness, those returning home from the hospital or rehabilitation facility, and those managing life with a chronic illness or living with a disability.

Hospice
Our program opens the door to a quality of life that may otherwise be unavailable for patients living with advanced illnesses. The primary goal of our services is to provide support, assistance, and guidance to the patient and their family — whether at home or at a long-term care facility. We understand that every moment is precious during this difficult time. Our focus is not only on medical support, but on the emotional, social, and spiritual needs of the entire family.

Adult Day Center
The Adult Day Center, a social adult day program model, offers a warm and supervised environment for older adults to engage in a meaningful day filled with activity and socialization.

Private Care
At Atlantic Private Care our home care nurses and aides provide one-on-one care in the NJ Counties of Morris, Essex, Union, Warren, Somerset, and Sussex that is designed to meet your needs.

Primary Care at Home
Internal Medicine Faculty Associates offer home visits through the Primary Care At Home program. We have nurse practitioners who make house calls to homebound patients 65 years or older, who live in the Morris County area.
