Alay Home Care provides Community Based Supports and Individual Supports through the DDD program in all NJ counties. We are able to provide assistance in the home or out in the community by matching a caregiver to the consumer or by hiring a family member, friend or someone they know.

Community-Based Supports
Services that provide direct support and assistance to consumers, in or out of the consumer's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Community-Based Supports may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.

Individual Supports
Services that provide direct support and assistance to consumers, in or out of the consumer's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Individual Supports may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.

Behavioral Support
Services that provide support with a behavioral function. Focused on the consumer improving their behavioral functions to a healthy and happy state. This service could tackle difficulties such as communication issues or a consumer's emotional and behavioral responses.

Career Planning
Career planning is focused on a consumer's goals and career plans, this can help them discover their wants and needs as well as qualify for employment or sustain self-employment.

Physical Therapy
Physical therapists provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities in patients with injury or disease. They restore, maintain, and promote overall fitness and health.

Occupational Therapy
Occupational Therapist is a profession that uses daily activities to help their consumers do what they want or need. They help consumers enjoy their lives to the fullest and promote a healthy lifestyle. 

In-Home Respite
In-Home Respite care is a service where consumers can remain at home while still receiving care, this can be on occasion or on a regular basis which can last from several hours to overnight.

Transportation
Transportation is a service that transports the consumer which can be done through Uber / Lyft, The consumer may choose if they want to be accompanied by a staff that uses Individual Supports/Community Based Supports or without a staff.
