RAPS (Regional Aftercare Program Services) assists adolescents (14-21) who have either current Division of Child Protection and Permanency (DCP&P) involvement or have a history of DCP&P involvement. Services include intensive case management and skill development training in the areas of education, employment, money management, identifying/accessing community resources, and problem-solving.
The program offers individual intervention, advocacy, and support. There are 10 slots available. LifeSkills is a 12-month program with a possible extension of 6 months with approval from the OAS.
All components of the LifeSkills program will be discussed with the young adult and the youth's voice will be captured in treatment planning and session notes. Goal planning will be a collaborative process between the Life Skills counselor and the young adult. Treatment planning and weekly sessions will be tailored to current needs, youth psychosocial history, and long-term goals. Treatment plans and interventions are youth-specific and customized to individualized strengths and needs
The program offers independent living skills training to youth living in Hunterdon County. There are no fees for this service.
Eligibility | Must be a resident of Hunterdon County; be between 14-21; currently have or have had DCP&P involvement; and be willing to participate in the RAPS program. |
