Bridgeway PACT teams provide integrated, comprehensive psychiatric rehabilitation services for people with serious mental illness. Bridgeway PACT teams believe that everyone can recover from the effects of serious mental illness when flexible,recovery focused services
and supports are available.
PACT
Services include:
- Individualized and flexible services mostly provided in the home or other community settings
- Individualized and flexible services mostly provided in the home or other community settings
- Medication prescription, administration, education and support
- Rapid and flexible response to potential crisis
- Career counseling, skills training and ongoing support to persons served on the job
- Dual Recovery Counseling
- Direct assistance with and teaching of critical life tasks (i.e. hygiene, cooking, budgeting, etc.)
- Assistance in developing peer and community social supports
- Partnership with family/significant others
- PACT services can be lifelong if needed
GOALS of PACT
- Self-management of mental illness
- Community integration
- Prevention of psychiatric
re-hospitalizations - Enhanced quality of life and positive
outcomes such as:
- Paid work
- Continuing education
- Supportive network of friends
- Positive family relations
- Living alcohol and drug free
* PACT services are not time limited and may be available as long as needed.
ELIGIBILITY REQUIREMENTS
Within the prior 18 months candidates
have one of the following:
PACT accepts referrals in Bergen, Essex, Hudson, Hunterdon, Passaic, Somerset, Warren and Union Counties. Please contact the Team in your county to obtain a referral form or to set up an appointment to get more information about Bridgeway’s PACT program.
Bridgeway Rehabilitation Services is a NJ State licensed and registered 501(c)3 nonprofit organization
Eligibility | Within 18 months prior to referral must have one of the following: 1. Two or more State Hospitalizations 2. One State Hospitalization with one or more other psychiatric hospitalizations 3. One State Hospitalization with multiple screening center episodes 4. Two or more STCF and/or County Hospital admissions 5. One STCF or County Hospital admission with one or more psychiatric hospital admissions, including voluntary admissions 6. One STCF or County Hospital admission with multiple screening center episodes 7. Two or more involuntary psychiatric hospital admissions at private psychiatric hospitals PACT candidates must have demonstrated lack of benefit from or not having participated in intensive ambulatory or residential mental health services for at least six months. PACT accepts referrals in Bergen, Essex, Hudson, Hunterdon, Passaic, Somerset, Warren and Union Counties.
