Phoenix Behavioral Health
We provide ISS (Individual Support Services), which is direct care and skill teaching in the youth's home and community. This includes personal assistance toileting, eating, bathing, dressing, and personal hygiene. Our services also offer parents and caregivers relief from assisting in these personal tasks for up to 12 hours per week.
For clients between the ages of 16-21 who are looking for more independence as they transition into adulthood, we offer teaching and mentoring to learn life skills such as money management, medication self-administration, housekeeping and grocery shopping, and accessing public transportation. This is a new service that can be requested through PerformCare or your Case Manager.
|Eligibility||Children eligible under the Children's System of Care between the ages of 5-21 with developmental disability needs.|
